Locum - Physician - Gastroenterology West Valley City UT

Holy Cross Hospital Jordan Valley and Holy Cross Jordan Valley west is looking for a gastroenterologist to help provide 24-hour call coverage to both sites concurrently.

The team will need a minimum of 7 shifts per month.

Schedule / Availability Requirements :

• 24-hour call coverage - 7a-7a
• 7 shifts per month minimum starting Sept 9th

Details :

• Setting : Outpatient, Call, Inpatient
• Trauma Level III
• Duties : EGD, EVS
• EMR : EPIC
• Support Staff : MA, RN, Echo Tech, Lab Tech, Nuclear Tech, X-ray Tech
• Reason for Coverage : Provider resigning.

EMR Required Training :

• Module (learn lessons) - Must be completed prior to virtual EMR training to attend the training.
• EMR Virtual training & Onsite training - Only able to attend if Modules are completed before training. Virtual and Onsite EMR training will be schedule the morning of providers first day.

Travel Preferences :

• Preferred Lodging : Preferred hotels
• Air Travel : Air travel okay
• Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations.

Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.

Credentialing Information :

• Timeframe : 60 days standard

• Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? No

Requirement description : SUBMISSION REQUIREMENTS

• Board Certified Gastroenterology - REQUIRED
• Active UT license - Required or IMLC (LOQ in hand at time of submission)
• Active BLS - REQUIRED for credentialing
• Gastro Fellowship - REQUIRED
• Active DEA - REQUIRED for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)
• Must stated # of shifts per month provider can offer to the team and must disclose month provider can start - REQUIRED
• Candidate must be available for a minimum of 7 shifts per month - REQUIRED
• Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - REQUIRED
• Clean Malpractice / Background - REQUIRED (Must include Self inquiry NPDB report)
