RN Case Manager, at Moms2B functions as the Nurse Educator to develop, teacheducational programs for the maternal and infant health curriculum in the Department of Obstetrics and Gynecology's Moms2B Program, a multi-site program that educates, supports and nurtures women through their pregnancy and their babies?
- first year of life in an attempt to reduce infant mortality in the State of Ohio; serves as lead clinical educational specialist;
- determines and develops educational programs in prenatal and postnatal maternal care and childbirth, education and counseling sessions for participants at various locations;
- develops, establishes and oversees education curriculum for healthy pregnancies for specific patient populations; plans, develops and implements personalized education programs based on needs assessments and of program participants;
- reviews and evaluates healthy pregnancy program curriculum; keeps abreast of new or revised trends in prenatal and postnatal care;
- participates in data research activities; provides summaries of training sessions conducted and metrics on maternal and neonatal mortality rates;
identifies and obtains needed clinical equipment and supplies; participates in fundraising and donor awareness activities as necessary.
- The RN Care Coordinator is : an advocate by ensuring access to appropriate care and services, promoting self-care management and prevention;
- a navigator, identifying and removing barriers to care and coordinating patient care services by utilizing the nursing process to help patients move through the healthcare continuum;
- a collaborator, working within the multidisciplinary team to ensure a comprehensive plan of care in accordance with nationally recognized evidence-based standards;
- an educator, instructing patients and their caregivers in understanding and managing their diagnoses, treatment, and symptoms;
- an expert, knowledgeable in care management principles and utilization of system and community resources tailored to meet individual patient needs;
and support NP provider practice. The RN Care Coordinator will manage a sub-population of patients in order to ensure the most effective and efficient use of health care resources to meet the patient's needs.
Minimum Education Required
Bachelor's Level Degree with a Major in nursing required
Minimum Qualifications
Bachelors degree in Nursing required. Masters degree desired. Current Ohio licensure, and current BCLS certification required.
Minimum of 3 years experience as an RN required. Experience in case management desired.
