Provide clients and family / caregivers teaching and education about health and wellness, safety factors, self-management, medications, self-monitoring, action plans, long-term planning, and how to access care.
Participate as a member of a multidisciplinary team by attending client meetings, consultations, internal team meetings, and external stakeholder meetings.
Assist with the development, implementation, and ongoing monitoring of Individual Service Plans by participation in client meetings, outreach to clients and caregivers, collaboration with healthcare providers, and making referrals.
Ensure program compliance with required health metrics by monitoring measures required by the state and connecting directly with clients, families / guardians, and team members to educate and problem-solve barriers.
- QUALIFICATIONS : Must be a registered nurse; a psychiatric nurse licensed as a registered professional nurse by the state where services are provided and certified by the American Nurses Credentialing Center (ANCC) as a psychiatric and mental health nurse;
- a PMH-APRN who is licensed as a nurse practitioner or clinical nurse specialist by the state where services are provided, who has graduated from a child and adolescent or adult psychiatric and mental health nurse practitioner or clinical nurse specialist program and is certified by the appropriate national certifying body;
