Job Overview : The Case Manager will be responsible for coordinating and providing comprehensive case management services to our clients.
This role involves assessing clients' needs, developing individualized service plans, and connecting clients with resources and support to achieve their goals.
The ideal candidate will have strong communication skills, empathy, and the ability to handle multiple tasks efficiently.
Key Responsibilities :
- Conduct comprehensive assessments of clients' needs, strengths, and goals.
- Develop, implement, and monitor individualized service plans.
- Provide ongoing case management and support to clients, ensuring their needs are met.
- Coordinate with various service providers and agencies to connect clients with necessary resources.
- Advocate for clients and assist them in navigating social services systems.
- Maintain accurate and up-to-date case records and documentation.
- Monitor clients' progress and make adjustments to service plans as needed.
- Provide crisis intervention and support as necessary.
- Participate in team meetings and case conferences.
- Stay informed about community resources and services available to clients.
Qualifications :
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field (Master's degree preferred).
- Previous experience in case management or a related field.
- Strong understanding of social services and community resources.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and as part of a team.
- Empathy, patience, and a genuine desire to help others.
- Proficiency in Microsoft Office Suite and case management software.
- Valid State driver's license and reliable transportation.
Preferred Qualifications :
- Bilingual in Spanish or Samoan language.
- Certification in Case Management (CCM) or related certification.
