Summary

SUMMARY : The Clinical Pharmacist Specialist is responsible for the provision of safe, effective, appropriate and cost-efficient medication therapy management to patients in the area of Ambulatory Care.

Under direction of the Director of Pharmacy and site-based Medical Director, they will assist in the planning, organizing and monitoring of the delivery of ambulatory care pharmacy services for the Alameda Health System.

An incumbent in the class of Clinical Pharmacist Specialist Ambulatory Care coordinates the pharmacy services within AHS and its clients.

In contrast to incumbents in the classification of Pharmacist who prepare and dispense drugs, the incumbent in the classification of Clinical Pharmacist Specialist Ambulatory Carehave advanced post graduate training or equivalent experience and possess a broad scope of skills and pharmacotherapy knowledge for providing direct patient care as well as providing complex, in-depth clinical services.

In addition to administrative duties, the Clinical Pharmacist Specialist provides consultation to the staff and to clients.

DUTIES & ESSENTIAL JOB FUNCTIONS : NOTE : The following are the duties performed by employees in this classification.However, employees may perform other related duties at an equivalent level.

1. Works with the System Director of Pharmacy to coordinate, improve, expand and deliver ambulatory care pharmacy services within AHS.

2. Monitors patients in a specific unit / specialty service, including collecting patient-specific data, interpreting drug-related laboratory data, performing relevant physical assessments, interviewing patients, and identifying patient goals of therapy.

3. Assesses patient-specific medical problems, prioritizes medical problems based on urgency and severity, and identifies preventive and health maintenance issues.

4. Evaluates patient-specific drug therapy by evaluating appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects.

Reports and documents drug errors, adverse drug reactions, allergies, and patient adherence issues.

5. Designs a comprehensive drug therapy and patient education plan for patient-specific problems and persuasively justifies recommendations based on patient-specific pharmacologic, pharmacokinetic, pharmacodynamic, pharmacogenomic, pharmacoeconomic, ethical, legal and evidence-based information.

6. Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, physical assessments as related to drug monitoring and adverse effect management, review of medication profiles and the patient medical records.

Documents clinical interventions in the electronic health record in a timely and professional manner.

7. Collaborates with patients, caregivers, and other healthcare professionals.

8. Develops and maintains the drug formulary for AHS as well as provides education for physician and other healthcare providers regarding additions and revisions to the formulary, including non-formulary request review.

Includes preparing drug monographs and cost analysis for presentation to committees, staff, physicians and other staff. Possesses and maintains knowledge of current evidence-based practice for applicable chronic disease states and serves as a drug information resource for other healthcare providers and team members.

9. Represents Clinical Pharmacy Services on various committees, staff meetings, and professional meetings as needed as it relates to the pharmacy area of practice / specialty.

10. Coordinates continuing education activities for medical and nursing staff; orients new employees on pharmacy practices and procedures.

Includes submissions to the pharmacy newsletters, participating in journal clubs, and providing in-services to professional staff.

11. Serves as a preceptor to pharmacy residents, students, and volunteers.

MINIMUM QUALIFICATIONS :

Education : Doctor of Pharmacy degree

Education : Completion of an American Society of Health-System Pharmacy accredited Postgraduate Year One (PGY1) Residency.

Education : Postgraduate Year One (PGY1) Residency or Postgraduate Year Two (PGY2) Residency in Ambulatory Care is preferred.

Minimum Experience : Three to five years of recent ambulatory care pharmacy experience preferred.

Licenses / Certifications : Possession of a valid registered pharmacist license to practice in California.

Licenses / Certifications : Become Basic Life Support (BLS) certified.

Licenses / Certifications : Board certification (BCPS or BCACP) is preferred.

Licenses / Certifications : National Provider Identifier must be obtained upon hire.

Additional Information

SAN position to backfill our 2.0 FTE of Hematology / Oncology Clinical Pharmacists during FMLA. This is a position intended to focus on Oncology in the inpatient and outpatient setting at Highland Hospital only.
