Responsibilities
- Coordinates all systems / services required for an organized, multidisciplinary, patient-centered care team approach.
- Helps ensure quality, cost-effective care for the identified patient.
- Manages the course of treatment of patients, coordinating care with physicians, nurses and other staff.
- Ensures quality patient outcomes are achieved within established time frames and with efficient utilization of resources.
- Conducts initial and ongoing assessments.
- Determines and manages outcomes.
- Ensures continuity of care through discharge planning.
- Functions as a contact person for patient, family, health care team members, community resources and employees as necessary.
- Ensures adherence to departmental policies and procedures.
Qualifications
- This position requires U.S. Citizenship for DOE contract security clearance
- Shall have a health professional-related degree, e.g. BSN
- Active state RN license
- Certification as a Case Manager (CCM) is highly desired
- 3 years of case management experience
All candidates must be able to :
- Sit, stand, walk, lift, squat, bend, twist, and reach above shoulders during the work shift
- Lift up to 50 lbs from floor to waist
- Lift up to 20 lbs
- Carry up to 40 lbs a reasonable distance
- Push / pull with 30 lbs of force
