Schedule Notes : This is for Bronx Borough. Patient range from 18 years old to 100 years old. THIS ROLE IS FOR TRUE AT HOME, nurse must travel to patient comes within the Bronx borough to provide care. This will be for the Bronx borough. Patient cases can range anywhere from 5-8 cases a day. Client is flexible with days of work it can be : Sunday to Wed, Thursday to Saturday etc. but must do consistent days. There is no overnight coverage. Position Summary : The RN Adult Care (Home Health) delivers skilled nursing care to adult patients (ages 18 100) within their own homes throughout the Bronx. This role involves independently traveling between multiple patient residences each day to provide individualized nursing assessments, medication administration, wound care, health education, and case management. The nurse plays a critical part in helping clients achieve optimal health outcomes, reducing hospitalizations, and supporting clients and caregivers with resources for continued care at home.

Key Responsibilities

Travel independently to provide in-home skilled nursing care according to physician orders and established care plans

Perform comprehensive patient assessments, including physical exams, history, vitals, and care environment analysis

Administer medications, manage wounds, perform basic lab tests, and monitor chronic and acute conditions

Educate patients and families on medication management, chronic disease, aftercare, and infection prevention

Document all assessments, interventions, and communications promptly using electronic health records

Collaborate with physicians, care coordinators, social workers, and other home health team members to ensure safe, high-quality, and effective care

Identify potential complications, respond to changes in patient status, and coordinate emergency services as needed

Support transitions of care (e.g., from hospital to home), make appropriate referrals to community or rehabilitation resources

Address social determinants of health and advocate to remove barriers to care when possible

Participate in ongoing quality improvement, compliance, and infection control initiatives

Required Skills & Experience

2 years of recent acute care experience (e.g., Med / Surg, Maternal & Child Care, Critical Care, Pediatrics)

Current experience in adult care and home health

Excellent time management, clinical assessment, and independent decision-making skills

Ability to travel by car or public transportation throughout the Bronx borough each day

Education : Bachelor's in nursing or Associates Degree (ASN, ADN) from an accredited Nursing Program (required)

Licensure & Certifications

Active New York State RN License (current and unencumbered)

Primary source verification of NYS RN registration

American Heart Association BLS (Basic Life Support) certification
