Job Description
- Facilitate communicationbetween the patient and the interdisciplinary team.
- Develop and maintain atherapeutic relationship by providing education and promoting self-advocacy andindependence.
- Utilize state-of-the-arttechnology to document skilled services provided.
Qualifications
- Licensed Practical Nursewith Valid CT LPN license
- One year of LPN experience;Homecare experience preferred.
- Valid driver’s license andreliable transportation required.
- Computer proficiencyrequired.
