Our flagship transitional work program Ready, Willing & Able integrates paid work, occupational training, and holistic supportive services to facilitate an individual’s return to mainstream society.

The Doe Fund has an annual operating budget of $65 million and a full-time staff 400+.

We are seeking a Case Manager / Substance Abuse Counselor for The Doe Fund's newest permanent supportive housing program for homeless men and women living with HIV / AIDS.

The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being.

Half of the residents will be referred by the New York City HIV / AIDS Services Administration (HASA) and the other half will be apartments for affordable to low-income households.

The Case Manager / Substance Abuse Counselor is responsible for providing supportive case management services to the HASA-funded residents.

The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV / AIDS population.

RESPONSIBILITIES :

  • Maintain a caseload and provide regular case management services to these individuals
  • Maintain resident records in compliance with agency and funding standards
  • Conduct and review assessments
  • Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self sufficiency
  • Monitor and ensure residents are linked to healthcare providers
  • Provide counseling and crisis intervention as necessary
  • Assist residents in securing and maintaining benefits
  • Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs
  • Facilitate groups and activities that promote health and independent living skills development
  • Monitor and assist residents with rent payments
  • Conduct regular unit inspections
  • Compile resources on substance abuse
  • Educate residents and staff about approaches to managing addiction
  • Oversee recovery readiness, relapse prevention, and recovery-oriented services
  • Organize sober recreational activities
  • Attend ongoing training to enhance the services provided to the residents
  • Assist with completion of HASA’s weekly and monthly reports as well as any other relevant reports
  • Other duties as assigned

QUALIFICATIONS :

  • Bachelor’s Degree in Social Work or relevant field
  • Credentialed Alcohol and Substance Abuse Counselor(CASAC or CASAC-T) certification or a License in Clinical Social Work (LCSW) is required.
  • Ideal candidate must have at least one year of experience working with people who are homeless, living with HIV / AIDS, have substance abuse histories or suffer from mental illness
  • At least 3 years of experience in substance abuse counseling
  • Knowledge of benefits in the New York City Metro area, HIV / AIDS, psychiatric diagnoses, substance abuse, and mental health issues
  • Solid computer skills internet, Microsoft Office, and charting database
  • Strong organizational, writing, and communication skills
  • Spanish speaking a plus, but not necessary
