Days : Monday, Tuesday, Friday, Saturday
Shift : 3 : 00 PM 11 : 30 PM
Guaranteed Hours : 32 hours / week
Requirements :
Certification : Valid Basic Life Support (BLS) certification required.
Experience : Prior experience as a Home Health Aide or in a similar caregiving role preferred.
Skills : Strong interpersonal skills, compassion, and the ability to work independently while following care plan instructions.
Physical Demands : Ability to lift and support participants, stand, and walk for extended periods.
Job Summary :
The Home Health Aide will provide essential home care and support to participants according to individualized care plans, ensuring that their medical, emotional, and daily living needs are met in a respectful and compassionate manner.
The role focuses on promoting comfort, safety, and dignity, while collaborating with a multidisciplinary care team.
Key Responsibilities :
Personal Care Assistance : Provide hands-on support with personal hygiene tasks, including bathing, dressing, grooming, toileting, and continence care as directed by the care plan.
Mobility Support : Assist with safe transfers, ambulation, and range-of-motion exercises to maintain participant mobility and prevent injury.
Medication Assistance : Assist with medication reminders and ensure that participants take prescribed medications on time, as per care plan instructions.
Health Monitoring : Regularly check and report any changes in the participant's condition (e.g., skin integrity, appetite, or overall health) to the supervising nurse.
Household Assistance : Help participants with light housekeeping tasks, meal preparation, laundry, and maintaining a clean, safe, and organized living environment.
Companionship : Provide emotional support and companionship to participants, engaging them in meaningful activities and conversations to improve their quality of life.
Documentation and Reporting : Maintain accurate and timely documentation of care provided, including any changes in the participant's condition, and communicate effectively with the healthcare team.
