Description
The License Practical Nurse coordinates care and oversees medical management of Pathways to Care residents, at the direction of the Clinical Services Coordinator.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Essential duties and responsibilities include the following :
General functions :
- Serve as a liaison to referring agencies, when needed to build an effective rapport, and representing the interests of residents, Pathways to Care and Catholic Charities of Central Florida professionally.
- Work in compliance with professional standards, state, and federal regulatory requirements.
- Understand and adhere to AHCA rules and regulations regarding medical oversight in an Assisted Living Facility. Work according to allowable activities under the facility’s license.
- Assist with AHCA, Dept of Health, Fire Department, and other entity monitoring’s.
- Oversee and audit use of the Medication Observation Record (MOR) including training of staff on proper use. Maintain accurate records of all prescriptions for residents.
- Represent Pathways to Care and Catholic Charities as a liaison with Seminole County Emergency Services and other government entities.
- Coordinate medical care of Pathways residents according to the directives of their physicians. Collaborate with other health care professionals and service providers to ensure optimal resident care, as needed.
- Work with volunteers as needed.
- Provide health education to residents regarding individual conditions and illnesses as needed.
- Provide training(s) to RA / CNAs on medical and facility topics.
- Maintain confidentiality of health and other resident information, following all HIPAA guidelines.
- Respond to emergencies timely with strong direction and leadership.
- Intervene when needed to resolve resident issues including responding to disruptive behavior and unplanned discharges.
- Ensure proper hygiene standards as required for the protection of residents, volunteers, and staff. Adhere to all safety regulations.
- Utilize computer, FAX, printer, telephone, scanner, etc. to carry out job duties.
Requirements
EDUCATION and EXPERIENCE
The PTC Nurse should be a licensed nurse in this state. Candidates should have at least two years of progressive leadership experience in a similar healthcare setting.
Current First Aid and CPR certification required for this position. Must have knowledge of medical terms to effectively communicate and coordinate with resident’s medical providers.
Have a working knowledge and experience using standard office equipment.
