Position Summary : This is an Advanced Practice Professional (APP) job opening, hiring Nurse Practitioner(s) and / or Physician Assistant(s) who will see a mix of Same Day and Collaborative Primary Care appointments.

Time will be split 50 / 50 between the Same Day Clinic and Primary Care. Both qualified / experienced NP and PA applicants will be considered.

This position will be located at Woodhaven.Advanced Practice Professional will provide health care and specialized health services to patients and families enrolled in Primary Care Medicine.

The APP will coordinate care of this patient population in collaboration with all members of the health care team. This position will be 100% outpatient.

APP will provide outpatient medical care by taking patient history, performing physical examinations, orders, and interpreting tests, making diagnoses, and institutes appropriate treatment programs.

Ideal candidates will have a minimum of 1-2 years’ experience as an APP, be able to see All Ages, and be comfortable supporting Adults undergoing Gender Affirming Care.

Experience providing Gender Affirming Care is not required.Independently evaluates patients in the clinic.Ordering / interpreting testing and lab results, modifying plan of care to address resultsProvide clinical expertise and direction to support patient medication managementMaintain accurate documentation regarding patient status, evaluations, and treatments to physicians, streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external team members and other ambulatory care providersPerform other related duties as required to assure patient safety and satisfactionParticipate in the development and presentation of educational experiences for patients, families, and health care professionalsProcedures specific procedures are individually credentialed as indicated by training and experienceSchedule Monday Friday, daysWeekend requirement Yes, 1 weekend morning a monthCall requirement- NAOR Time- NADuties & Responsibilities : In collaboration with physician(s), APP will provide independent acute and / or routine medical care to a patients and families enrolled in Primary Care MedicineWrites appropriate orders, if / when applicablePractices safety, environmental, and / or infection control methodsCoordinates the activities of patient care and support staff within the clinicWrites progress notes in EMR indicating patient status and treatment procedures performedProvides health education to patients, families, and / or community health practitioners and groupsEnhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshopMay perform clinical research related to specific operationsPerforms miscellaneous job-related duties as assignedKnowledge, Skills, and Abilities : Knowledge of clinical operations and proceduresKnowledge of drugs and their indications, contraindications, dosing, side effects, and proper administrationAbility to maintain quality, safety, and / or infection control standardsAbility to perform medical examinations using standard medical proceduresLife support as required through privilegingAbility to clearly communicate medical information to professional practitioners and / or the general publicAbility to educate patients and / or families as to the nature of disease and to provide instruction on proper care and treatmentKnowledge of current and emerging trends in technologies, techniques issues, and approaches in area of expertiseAbility to communicate therapeutically with patients and familiesAbility to react calmly and effectively in emergency situationsKnowledge of related accreditation and certification requirementsAbility to make administrative and procedural decisionsSkill in preparing and maintaining patient recordsKnowledge of primary care principles and practicesConditions of EmploymentEmployee Health requires all employed providers to complete a health screen per HFHS policy.

Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipmentWork is normally performed in a typical interior / office work environmentWill work with blood or blood-borne pathogens and will require OSHA trainingGENERAL SUMMARY : Provides inpatient and / or outpatient medical care.

Takes patient history, performs physical examination, orders, and interprets tests, makes diagnoses, and institutes appropriate treatment programs.

Works in collaboration with staff physicians, house officers, nurses, and other medical care team members to ensure that patient outcomes are achieved safely and efficiently.

Specific procedures are individually credentialed as indicated by training and experience. EDUCATION / EXPERIENCE REQUIRED : Master's degree in nursing.

CERTIFICATIONS / LICENSURES REQUIRED : Certified Nurse Practitioner. Licensed as a Registered Nurse. Current BLS or ACLS as required.
