Sentara RMH Medical Center in Harrisonburg, VA is hiring a Community Health Project Manager .

The 238-bed community hospital has been serving Harrisonburg, Rockingham County and surrounding areas since 1912, then partnered with the Sentara Health S ystem in May of 2011.

Sentara RMH features the RMH Hahn Cancer Ce nter, a state-of-the-art center equipped with the latest cancer fighting technologies available in the nation administrating more than 18,000 cancer treatments.

Job Description-

Leads and / or supports the strategic planning, implementation, and evaluation of initiatives that measurably improve health and well-being in underserved communities.

Directs a portfolio of initiatives that connect clinical care with community-based programs to improve health outcomes and social determinants of health in alignment with prioritized community needs.

Serves as a SRMH liaison for key community partnerships and facilitates connection with internal hospital stakeholders to deploy resources toward community activities that align with identified priorities.

Job Requirements-

Education-

Minimum required : Bachelors degree public health, health sciences, health administration, social work or related; Masters preferred.

License / Certification-

At least 2 years of experience in health care, project management, and monitoring and evaluation.

Hours- Monday-Friday- Normal business hours
