Job Description
POSITION SUMMARY : Responsible for planning and organizing Individual and group activities to meet the social, emotional, intellectual, and spiritual needs of residents.
Communicates available programs to residents and encourages participation.
REPORTS TO : Administrator / Marketing Admissions Director
RESPONSIBILITIES :
1. Adheres to and conveys a philosophy that supports the dignity, privacy, Independence, choice, and Individuality of residents.
2. Conducts an assessment with each resident to determine those activities In which
resident would be most interested in pursuing and / or participating based on resident schedule.
3. Summarizes information obtained from each resident's activity and Interest profile to determine which activities are of Interest to the greatest number of residents.
Plans and organizes programs, events and a.ctivities around resident's interests.
4. Leads and / or oversees individual and group activities on a daily basis, encouraging resident participation.
5. Develops and distributes monthly activity schedules and posted for easy reference by residents.
6. Develops and distributes a newsletter on a regular basis for resident's family members and selected referral sources.
7. Acknowledges birthdays, anniversaries, and special accomplishments by planning a special event and / or giving the resident a card, note, or small gift.
8. Orders needed supplies (e.g., art and craft supplies, party supplies, party supplies, etc.).
QUALIFICATIONS :
1. Able to lead and motivate people; excellent Interpersonal skills.
2. Good planning and organizational skills.
3. Able to communicate well, both orally and in writing.
4. Self-starter, able to carry out diverse duties with minimal supervision.
5. Enjoys thinking of new Ideas; is creative and resourceful.
6. Good computer skills (i.e. MS Word, Printshop, etc.).
7. Holds a valid driver's license ofthe appropriate type and a driving record which meets the community insurance standards.
8. Ail general staffing requirements.
9. General understanding of the needs of the elderly and willingness to learn. DUTIES : Performs all duties within the guidelines of Carecore Montgomery .
1. Assess and complete the sections of the Minimum Data Set (MDS) assigned completely,
timely and accurately.
2. Use the resident assessment protocols (RAPS) to determine whether to proceed to care planning for concerns related to a resident's activity program needs.
3. Identify and document in the resident's care plan , the activity interests , needs , and
concerns of the resident. Care plans are reviewed and updated as necessary but at least quarterly.
4. Participate as an active member of the interdisciplinary team and attend all resident care conferences.
5. Plan and conduct Resident's Council meetings (Tea's).
6. Plan and implement evening and weekend functions as necessary.
7. Take part In-service education programs and attend all in-service sessions relating to
Activity Program.
8. Recruit , schedule and supervise assistants and volunteers.
9. Organize and schedule community events.
10. Plan , schedule and implement room visits and in-room activities for residents unable to leave their rooms.
11. Maintain an activity attendance record for each resident.
12. Assess needs and maintain an adequate supply of materials to implement activity programs.
13. Accept assigned duties in a cooperative manner.
14 . Follow all facility policies and procedures.
15. Assume Quality Assurance duties as assigned.
16. Come to work in neat , clean attire and consistently present an appropriate professional appearance.
17. Professional appearance and attitude.
18. Come to work as scheduled and consistently demonstrate dependability and punctuality.
19. Consistently work cooperatively with residents, resident representatives , facility staff,
physicians and ancillary service providers.
20. Ability to understand and comply with applicable Federal , State and local regulations and requirements.
