Position Summary :

Liberty Resources, Inc. is seeking a Board-Certified Physician for our newly designated Federally Qualified Health Center (FQHC) located at 1045 James St.

Syracuse NY. We are seeking a dynamic Medical Director to share in our Vision for delivering high quality healthcare in our integrated primary care and behavioral healthcare practice.

The Medical Director will play a pivotal role in providing primary care while also overseeing administrative functions to ensure the smooth operation of the practice.

Working collaboratively with our Medical Director for Behavioral Health and other clinic leadership, this position offers a unique opportunity to make a significant impact on both patient care and organizational development in an integrated care environment.

As Medical Director, you will have a significant role in shaping the Vision and delivery of our Integrated Healthcare clinic services as part of the leadership team.

Liberty is also designated as a Certified Community Behavioral Health organization, one of only 62 dually certified CCBHC and FQHC’s in the Nation.

This distinction affords the Medical Director a unique opportunity to help transform care delivery in a highly supportive and integrated system of care.

These designations also allow for a richly reimbursed delivery system. This allows practitioners to focus on pt. care instead of service volumes.

While the Medical Director is responsible for direct pt. care, as our practice grows, administrative duties will increase, and direct pt.

care will decrease over time and be provided by other providers. We have both a strategic Vision and Plan for growth. We also pride ourselves on having a culture of being Family supportive where work-life balance is important for our employees.

The position is based in our Clinics at 1045 James St. Syracuse, M-F with no evening or weekends.

Compensation and Benefits : We offer a highly competitive compensation package including :

  • Minimum base compensation of $325,000 plus DOE.
  • Opportunity to participate in incentive program(s).
  • Employer match 401(K), tax advantaged 457(b) retirement plan, and supplemental retirement program for highly compensated individuals.
  • 4 weeks paid vacation with 8 days paid personal leave plus accumulated sick leave.

Responsibilities :

Clinical Practice (50%-60%)

  • Provide direct patient care for 50% - 60% of working hours (the level of clinical work will be evaluated every 6 months as the clinic continues to grow and evolve).
  • Deliver comprehensive medical services to patients based on the area of specialty (internal medicine, family medicine Med / Peds),
  • Collaborate with behavioral health team members to provide integrated care for patients with co-occurring medical and behavioral health conditions.
  • Demonstrate clinical leadership and serve as a role model for the highest standards of evidence-based practice and patient-centered care.
  • Participate in regular case conferences and interdisciplinary team meetings to coordinate care plans for complex patients.
  • Participate in backup telephonic on-call duty (Liberty Primary Care has a service that is paid to take first call) .

Administrative Duties (40%-50%)

  • Oversee primary care clinical operations providing direction, guidance, and leadership while fostering a culture of excellence, and patient-centeredness.
  • The Medical Director is responsible for providing both administrative and clinical care oversight in a Federally Qualified Health Center Look-a-Like (FQHC).

Provides both oversight of the primary care of the FQHC-LaL , and the care of uninsured and underinsured patients with diverse medical conditions.

  • Play a key role in building a highly effective, respectful, and kind primary care multidisciplinary team.
  • Provide clinical leadership and direction to Primary Care quality initiatives in compliance with all regulatory standards.

Partner with Liberty leadership to lead the Quality Assurance and Incident Management processes to monitor and assess the quality and appropriateness of patient care and clinical performance, to identify and resolve problems, and to pursue opportunities to improve patient care.

  • Supervise Advanced Practice Providers (APPs), providing mentorship, guidance, and support in clinical decision-making.
  • Guide the development and implement practice policies, protocols, and best practices to enhance patient outcomes and satisfaction.
  • Collaborate with executive leadership and program leaders to develop strategic plans and initiatives for practice growth and effective integrated care models.
  • Serve as a liaison between clinical and administrative staff, and the organization as a whole, to facilitate effective communication and teamwork.
  • Offer clinical perspective regarding Health Care Information Technology priorities and implementation.
  • Act as a liaison with local, regional, and national clinical organizations.
  • Play a key role in developing educational programs at Liberty for multidisciplinary students as appropriate (APPs, Med Students, Residents).
  • Demonstrate financial stewardship of practice and health system resources by participating in the budget planning and making recommendations pertinent to medical activities including plans and projections for staff support and equipment as appropriate.
  • Oversee and participate in medical chart reviews to fulfill regulatory requirements.
  • Establish measurable performance expectations (productivity, quality of care, timely completion of medical records and patient care, charting, and other administrative duties, etc), provide supervision, coaching, counseling, evaluation, and disciplining of providers and other clinical primary care staff (as necessary).
  • Participate in and provide clinical perspective to strategic planning, annual planning and day-to-day decisions (e.g. clinical tools, equipment, and practice vendor relationships)

Qualifications :

  • Medical degree (MD or DO) from an accredited medical school.
  • Active and unrestricted medical license in NYS
  • Minimum of 5 years of clinical experience in primary care practice
  • Previous leadership experience in a clinical setting
  • Strong interest in integrated care models and experience working collaboratively with behavioral health providers
  • Demonstrated ability to lead change, drive innovation, and promote a culture of continuous improvement
  • Commitment to delivering compassionate, patient-centered care, and advocating for the needs of underserved populations

May qualify for National Health Service Corps (NHSC) and Public Service Loan Forgiveness (PSLF) Loan Repayment Programs.
