Responsible for social work evaluation of patient / family psych / social and environmental needs, as well as implementation of the social work treatment plan and documentation consistent with insurance guidelines.
Work is evaluated by Hospice Manager through patient record documentation, verbal communication and direct observation.
Work is normally performed in a home setting. Requires considerable travel. Occasional work is performed in an office setting.

Required qualifications :
- Master's Degree MSW from a school of Social Work.
- Upon hire : Washington Advanced Social Worker License Or Washington Advanced Social Worker Associate License Or Washington Clinical Independent Social Worker License Or Washington Clinical Independent Social Worker Associate License
- National Provider Identifier (NPI) number within 30 days of hire
- 1 year Experience in a health care setting.
- 3 years Post Master's work experience, or prior experience in hospice care setting.
Preferred qualifications :
- 2 years' experience in a health care setting.
- Prior experience in home health / hospice care setting.
