As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager you will support some of our most complex patients, assisting them in navigating a challenging healthcare system.

Through medical record review and consultation, you will identify the medical, social, emotional, and financial needs of your patients with CKD / ESKD and implement appropriate interventions.

You'll work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent, cost-effective care.

This Registered Nurse, RN Case Manager functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESRD and CKD patients, and provides coordination of care between the patient, family, physician, provider and care teams, the community, and Regional Operations Manager.

The position will assist some of the most complex patient population in navigating the health care system and making it work for them.

You are able to support and look at a patient holistically, helping them and their whole care team, navigate the health care system.

You are their advocate and get 1 : 1 time to assist in impacting the patient experience.

This role helps to support patients in the Bay area, including the Vallejo and Benicia area, for both in-person and telephonic support pending patient needs.

Position Details :

  • Location : Hybrid-Remote. Work from home (telehealth) and travel within an assigned geography for in-person visits (dialysis clinics and nephrology practices).
  • Monday- Friday schedule with the ability to accommodate patient and nephrology partner's availability. Must be flexible.
  • Full-time position.
  • Quickly build empathetic relationships with patients and families.
  • Coordinate care for patients' care including care transitions, management of complex / at risk patients, managing ongoing needs and establishing a treatment plan in partnership with the care team.
  • Identify and address social determinants of health including medical, emotional, education, community, and financial concerns.
  • Coordinate for streamlined / effective transition of care when needed.
  • Assesses patients and work to identify unreported medical conditions or changes that may lead to adverse outcomes.

Qualifications for Registered Nurse, RN Case Manager / Requirements :

  • A.D.N degree from accredited school of nursing required; B.S.N preferred
  • Current Registered Nurse (RN) license in the state of practice required
  • Continuing education credits maintained as required by state of practice required
  • Minimum of five (5) years' experience in clinical nursing is strongly preferred
  • Demonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomes
  • Experience and passion for educating patients
  • Current CPR certification required
  • Ability to adapt to new / changing technology
  • Current driver's license in state employed with positive driving record and able to meet requirements of insurance coverage required
  • Basic computer skills and proficiency in MS Word and Outlook required
  • Passion for delivering care to patients with complex, chronic illnesses.
  • You are flexible and are willing to pivot quickly on priorities.

Preferred Qualifications :

  • 2+ years of Case Management or Chronic Care Management
  • 1+ years of utilization management experience
  • Spanish-speaking preferred in some markets
