Job Summary

This role is a key leader of the integrated care team and provides clinical and performance oversight of a team that is comprised of licensed social workers, licensed behavioral health clinicians, complex care managers and social determinants of health specialists.

This role understands the complexities of co-occurring conditions and social determinants of health (SDoH), how they impact the overall health and wellbeing of our members and strategically identifies and implements integrated processes to improve outcomes.

The Manager is responsible for the daily operations of the team based on key performance indicators that include reducing potentially preventable events (e.

g., ER visits, hospital admissions (medical / psychiatric, and readmissions), increasing utilization of long acting injectables, increasing member engagement with community resources, complex care management requirements and member retention.

The Manager plays a key role in the Collaborative Care processes that integrate primary care with psychiatric services. This position works closely with the center's triad leadership structure that includes the medical director, practice administrator and market general manager to meet Company and market level goals.

Oversight functions include department training / onboarding, performance management, clinical oversight / coaching, and case management to NCQA, regulatory, and health plan standards.

This role serves as a behavioral health subject matter expert (SME) across the company for both internal and external facing meetings and initiatives.

The Manager partners with counterparts on the medical team and psychiatric provider(s) to deliver integrated care within the collaborative care model.

Duties and Responsibilities

  • Lead a high performing team comprised of licensed clinical staff and unlicensed staff.
  • Closely collaborate with medical counterparts and psychiatric provider to deliver integrated care under a collaborative care model through interdisciplinary care team meetings, case consultations, etc.
  • Work closely with the National SMEs to analyze data and design, implement and measure processes that increase positive health outcomes as evidenced by the reduction in overall health care cost and decreased unnecessary utilization.
  • Oversee clinical staff development and performance that results in person-centered care planning, SMART goals, clear and consistent documentation, use of evidence-based therapeutic approaches to care including motivational interviewing, trauma informed care, harm-reduction, cognitive-behavioral therapy.
  • Oversee non-clinical staff development and performance that results in increases member engagement in treatment, reduces health care costs, and increases member access to housing and community resources.
  • Manage complex care management caseloads to ensure member follow up, timely and accurate documentation, and compliance with company, health plan, and NCQA requirements.
  • Facilitate and document outcomes of interdisciplinary care team (ICT) meetings and ensure follow up is completed and documented for BH related action items.
  • Serve as a SME on clinical techniques including motivational interviewing, harm reduction, ACEs and trauma informed care to lead the training and adoption of these approaches to member care for the assigned market, and in new markets, as needed.
  • Work with the Resource Specialist to establish and maintain relationships with community-based organizations to address the SDOH needs of members in the local market.
  • Provide clinical crisis intervention and de-escalation for members presenting in a crisis situation (via telephone or in person), including presentation of suicidality, risk of harm to self or others, or psychosis with agitation, and disposition as clinically appropriate;

supervise related staff; and conduct debrief sessions post-intervention working closely with the medical director.

  • Act as back-up for decision-making and procurement for the Member Fund when the Resource Specialist is unavailable.
  • Work closely with the Resource Specialist and the National SDOH Program Manager to develop a local strategy to address the social needs of members.
  • Proactively identify and mitigate barriers to team productivity and member care through data analysis, creative problem solving, and process optimization.
  • Use data and operational reports to positively impact utilization, health outcomes, quality metrics, and member satisfaction / retention to care.
  • Meet deadlines and manage competing priorities.

Minimum Qualifications

  • Clinically licensed in the state of practice with 5+ years of supervisory experience in a behavioral health related field including social work, psychiatric nursing, mental health, addiction.
  • Licenses may include : LCSW-C, LPC, LPCC LCPC, LMHC, RN
  • Qualified clinical supervisors will be given preference
  • 3+ years of experience in a community mental health center, managed care organization, FQHC, inpatient psychiatric facility or other setting that serves clientele with complex behavioral health needs.
  • Experience with complex government-sponsored populations preferred, e.g., Medicaid, Medicare beneficiaries
  • Experience with member engagement, transitions of care, clinical care, and / or case management
  • Advanced computer skills with proficiency in Outlook, Word, Power Point, Excel.
  • Experience working in multiple electronic health records.
  • Excellent oral and written communication skills with ability to format and present to internal and external stakeholders.
  • Excellent leadership skills with proven record of developing and maintaining team morale, high productivity levels, and minimal undesirable staff attrition.
  • Excellent organizational and time management skills to prioritize and meet deadlines.
  • Ability to analyze data to identify and implement process improvements.
  • Passion for addressing health disparities and social needs of low-income, high-risk populations of diverse backgrounds, ages, and abilities.
  • Operates personal motor vehicle.
  • Proven record of ability to lead through change in a dynamic environment.
  • Hold and maintain active driver's license and proof of insurance in state of practice.

Working conditions

This job operates in the community and within a professional office environment and routinely uses general office equipment.

This role requires reliable transportation to commute back and forth between inpatient facilities and office; and routinely uses general office equipment.
