Physical Therapist - Home Care
We are seeking a Physical Therapist to join our team at Bridge2 Homecare, LLC! The PT will provide professional services within the scope of physical therapy practice standards in collaboration with primary care physicians and other members of the health care team.
Pay Rates : $90 Start of care; $70 per visit.
Responsibilities :
- Receive the intake referral information and prepares PT paperwork / tools necessary for the visit.
- Obtains all pertinent medical history from patient, family or significant others.
- Performs PT assessment and documents accordingly.
- Performs home safety check and environmental assessment of the patient's home environment.
- Assists the physician in evaluating the level of function.
- Helps develop an appropriate and effective PT plan of care (POC) to be submitted to the physician for approval and implementation, revising as necessary and at least every 30 days.
- Functionally reassess the patient for on-going therapy.
- Determines medical necessity for other services.
- Evaluates the patient's ADL and iADL abilities.
- Effectively communicates ongoing with patient and family regarding the POC and progression of such.
- Effectively instructs patients and families in the safe use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, mechanical lifts etc.).
- Effectively communicates with other disciplines in the case.
Qualifications :
- Successful completion & graduate of a PT program approved by the commission on Accreditation in PT education (CAPTE) or successor organizations of the CAPTE.
- Current, active license to practice physical therapy in MA.
- Minimum two (2) years' experience as a physical therapist.
- Criminal background check & Current CPR certification.
Pay : From $70.00 per hour
