About The Position
Job Title : Registered Nurse Case Manager

Job Description :
Our client is seeking a dedicated Registered Nurse Case Manager to join their team. The ideal candidate will have a strong background in case management and acute inpatient care, with a deep understanding of illness characteristics and treatment expectations.
This role requires excellent communication skills and the ability to efficiently utilize resources to promote quality, cost
- effective outcomes. Responsibilities :
- Work a 40
- Work a 40
- hour week, with shifts from 7 : 30 am 5 : 30 pm. Start times may vary. No major holidays required.
- Provide coverage every 6th weekend as needed.
- Assess, plan, implement, monitor, and evaluate the options and services required to meet a patient's health needs.
- Manage the length of stay and promote efficient utilization of resources.
- Determine medical necessity and appropriateness of admission with InterQual screening and inpatient list from Medicare.
Requirements :
- Minimum of 3 years of experience in case management, with a strong understanding of current MS patterns of practice.
- Knowledge of benefits, plan contracts, eligibility issues, and requirements.
- BSN certification is required.
- Preferred : 5+ years of experience with UM & CM, Epic.
- Preferred : CCM or related certification.
