No on-call, no weekends and no holidays required.
Primary Responsibilities :
- Conduct in-home assessments including anevaluation of past medical history, review of systems, medicationreconciliation, vital signs, and comprehensive physical exam
- Perform evidence-based practice screeningsincluding point of care testing (as appropriate)
- Identify diagnoses to be used in care management andactive medical management in the furtherance of treatment
- Formulate a list of current and past medicalconditions using clinical knowledge and judgment and the findingsof your assessment
- Communicate findings inyour assessment that will be used to inform member's PCP ofpotential gaps in care
- Identify urgent andemergent situations and intervene appropriately
- Educate members on topics such as disease process,medication, and compliance

RequiredQualifications :
- Active NP licenseor ability to obtain by start date. Licensure must be unencumbered,free of any open / unresolved disciplinary actions includingprobation or restrictions against privilege to practice
- Active ANCC or AANP national certification in Family,Geriatrics, Adult, Adult-Gerontology Primary Care, orAdult-Gerontology Acute Care certification, or the ability toobtain national certification and / or NP license in state ofassignment by start date
- Ability to completephysical requirements of the job including lifting a 30-pound bagin and out of the car, navigating stairs and a variety of dwellingconditions, and sit, stand and kneel to perform physical assessment
- Reliable transportation to complete homevisit assessments
PreferredQualifications :
- 1+ years ofclinical experience in their highest level of education, clinicalsetting
- May be requested to obtainadditional licensure in other geographic areas
- Experience in gerontology, cardiology, internalmedicine, or endocrinology
- Home Health careor home visit experience
- Ability to travelthroughout the state, beyond your assigned region, up to 5% of thetime. Based on business needs with advanced notice, eligible foradditional compensation incentives
- Fluencyin Spanish, Cantonese, Korean, Vietnamese, Polish, or otherlanguage
