About This Hospice Triage Registered Nurse Position (RN)
HopeHealth is seeking a Response Team Triage Registered Nurse for its affiliate HopeHealth Hospice & Palliative Care.
The Triage Nurse will answer telephone calls from patients and families and refers and provide support or guidance to the caller.
Calls may be resolved over the phone, or the Triage Nurse may determine to schedule a Registered Nurse to visit the patient.
This is a part-time 24 hour day position and will be on-site, at our Providence, RI location.
The schedule for this position is 7 : 00 am - 3 : 30pm working every third weekend
This position is eligible for a bonus of up to $2,500.

What will you do?
Acts as a communication liaison between care providers, professional staff, patients, referral sources, insurers, and physicians, e.
g., voice mail report, answering service reports. Maintains close communication with the Patient Care Manager (PCM) especially regarding unusual situations.
- Establishes and maintains positive relationships with customers. Handles concerns / complaints in a timely manner with assistance and guidance from the Patient Care Manager.
- Teaches and supports patients, families, and facility staff over the telephone.
- Assists with symptom management insuring patients / families care is communicated and continued 24 hours / day, 7 days / week by utilizing off shift personnel to make visits and follow up calls as needed and under guidance of PCM.
- Ensures patients and families have strong support after normal business hours through competent telephone response and assistance.
- Responds to all patient calls offering visit from nurse, social worker, chaplain as appropriate. Prioritizes and schedules staff visits ensuring emergent visits take high priority.
Teaches and supports patients and families and facility staff over the telephone.
What are the qualifications for this job?
- RN required BSN preferred
- RN License in RI and MA is required
- 1-3 years Registered Nurse experience with hospice patients
- Strong nursing assessment, clinical skills, and communication skills
