Job Title : Rehabilitation Therapist Job Specialty : Speech Therapist Job Duration : 13 Weeks Shift : Monday to Friday Guaranteed Hours : 32 Hours Per Week Experience : SNF Experience Preferred;
New Graduates Welcome License : State License Required Certifications : None Specified Must-Have : Completed prescreen- Resume and verified license attached Job Description : Ensure prescreen is completed and attached for candidate submission.
- Attach resume and license verification; incomplete submissions will be declined.- Conduct a COVID test 72 hours prior to the start date with results available 48 hours before.
- Review and comply with the credentialing checklist including : - State License Verification - Resume and References (3 required) - County Criminal Check (7 years for all counties resided and worked in) - Federal Criminal Check (7 years or more) - Multi-State Criminal Search (7 years or more) - Nationwide Sex Offender Search (7 years or more) - HHS / OIG Search - GSA Search - Social Security Number Trace - 2-Step PPD or Quantiferon, or proof of positive PPD and Chest X-Ray - SAM Search - State Background Check (facility run)- No holiday coverage required.
