NP, Nurse Practitioner/Pa, Physician Assistant - PrimaryRural Health

Summary :
Responsible for the overall comprehensive care provided tothe patients in the CHRISTUS SETX Health System. NP will coordinatecare with the Medical Director utilizing training with the scope oftheir practice, conduct patient education and ensure accurate andtimely documentation in accordance with the regulating agencies andorganizational policy.
Will also participate in community agenciesand organizational policy; participate in community events andmarketing opportunities involving the CHRISTUS SETX Health System.
The NP will coordinate patient management and individualize a planof care for each patient, working closely with the Medical Directorand other team members.
She / he will takemedical histories, perform physical exams, order and interpretlaboratory studies and provide treatment to patients with chronicdiseases.
Working closely with the RN and Medical Director toensure follow up calls are made, assist in identifying additionalvisits or interventions that may be needed as well asmanaging / adjusting medications and educatingpatients.
Requirements :
- Current Licensure through the Texas Board ofNursing OR Licensed Physician Assistant by the Texas State MedicalBoard or the ability to obtain prior toemployment
- Valid DEA license or the ability toobtain one prior to employment
- BLS obtained bythe American Heart Association
