Title : Intensive Health and Wellness Medical Case Manager, Ending the HIV Epidemic (EHE)
Summary
EHE Intensive Health and Wellness Medical Case Manager position provides holistic, client-centered case management services to individuals living with HIV. The role emphasizes health promotion, behavioral health integration, and linkage to supportive services that address the social determinants of health (SDOH). Through coordinated care and wellness coaching, the Case Manager promotes treatment, adherence, viral suppression, and overall well-being.

Required Education and Experience
Bachelor’s degree in human service field; good computer communication, interpersonal, and writing skills are required as is ability to work independently. Job involves local travel is required so a valid driver’s license and vehicle insurance is required.

While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
