Manager, Integrated Health Management Services (Honolulu)
- Select, train, and manage designated staff, fostering a work environment that encourages teamwork, promotes personal growth and learning, couples empowerment with accountability, learns from failure and celebrates success.
- Manage all case management / health coordination activities on their designated island(s) ensuring all assessments, health action plans / care plans are completed and implemented according to Med-QUEST, Medicare, or commercial line of business requirements.
- Administer an effective program through multi-disciplinary teams with the goal of improving health care outcomes and member satisfaction.
- Identify opportunities and develop programs to improve the operational efficiency and cost-effectiveness of case management / health coordination processes and programs.
- Oversee the performance of their team on their assigned island to ensure that appropriate services are being provided. The manager will maintain a positive working relationship and ensure that deliverables are being met in accordance with contract requirements.
- Establish and maintain cooperative and productive professional relationships with health coordinators and case managers at various facilities and community organizations.
- Act as a liaison and mediator between participants, families, and service providers addressing any concerns that may arise.
- Attend and participate in meetings / workgroups / committees for community organizations and serve as resource / liaison between HMSA and the community.
- Performs all other miscellaneous responsibilities and duties as assigned or directed.
