This position will train, support, and perform annual wellness visits within the offices of Independent Physician Associates and Advanced Practice Providers on Medicare documentation guidelines, HEDIS / Quality measures, and clinic workflows that align with Value Based Care delivery models.
There would also be one day per week seeing patients in a Centerwell owned Senior Clinic.
Working with IPA Providers / APPs to create and / or enhance workflows, close quality gaps, improve identification and capture of HCC codes, improve medication adherence, and identify high risk and emerging risk patients to enhance patient outcomes.

Patient Care
Working with IPA Providers and Advanced Practicing Providers (APPs) to improve quality, care management, and create or enhance current workflows, while using evidenced based practice, to improve care delivery, documentation, reimbursement, and patient outcomes.
Thus, demonstrating commitment to improving clinical practice and increasing revenue.
Analyzing Key Performance Indicator metrics utilizing EMR and other relevant reports while collaborating with population health and the quality department to improve various workflows to enhance patient outcomes and improve metrics
Working with IPA Providers / APPs to create and / or enhance workflows, close quality gaps, improve identification and capture of HCC codes, improve medication adherence, and identify high risk and emerging risk patients to enhance patient outcomes.
Working with current Provider Engagement Specialists in Creating and distributing monthly medical informatics, adding their clinical expertise.
Providing elbow support and training to IPA Physicians and Advanced Practice Providers on Medicare documentation guidelines, HEDIS / Quality measures, and clinic workflows that align with Value Based Care delivery models.
Participates in potential growth opportunities for new or existing services within the IPA.
Assures personal compliance with licensing, certification, and accrediting bodies.
Required Qualifications
Minimum of three years directly applicable experience required
Successful completion of a Nurse Practitioner / Physician Assistant program with Board Certification
Active, unrestricted Nurse Practitioner / Physician Assistant license in state of practice location
Active and unrestricted DEA license
Excellent communication skills; written and verbal, demonstrate a high level of skill with interpersonal relationships and communications with all colleagues;
fully engaged in the concept of 'Integrated team-based care'
Experience or willingness and ability to learn / adapt to practice in a value-based care setting
Superior patient / customer service
Basic computer skills, including email and EMR
Appropriate certification to write prescriptions under the authority of a collaborating physician / medical director
This role is considered patient facing and is a part of our Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB
Preferred Qualifications
Medicare Provider Number / Medicaid Provider Number
Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value based relationship environment
Knowledge of Medicare guidelines and coverage
Knowledge of HEDIS quality indicators

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$102,200 - $140,700 per yearThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and / or individual performance.
