SUMMARY OF POSITION :
Under the direction of the Program Coordinator, the HIV Navigator / Case Manager will provide direct and indirect services to support follow-up, linkage and referrals, and data entry for HIV, behavioral health, and substance use-preventative care and care to individuals living with HIV or with substance misuse issues.
The HIV Navigator will be the front-line staff involved in client engagement and building strong relationships with HIV-positive clients.
The HIV Navigator will help clients stay engaged in medical care and adhere to their medications. They are committed to removing the client’s barriers to care by identifying critical resources for clients, helping them navigate through healthcare services and systems, and promoting client health.
They are committed to building strong relationships with community partners that may include hospitals, FQHCs, community-based, and supports to foster positive client health outcomes.
The HIV Navigator / Case Manager will work closely with participants who are 13-18 years old, identified as LGTBQ or high risk, and living in the West Chicago area to provide follow-up to HIV testing and counseling, behavioral health and substance abuse screening and, administer questionnaires, enter data and provide and track referrals as needed.
The HIV Navigator / Case Manager will maintain client confidentiality, follow HIPPA regulations, and respectfully represent PRCC and the designated programs in the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES :
- Provides support, advocacy, information, resources, and referrals to participants to ensure linkage to care; acting as a liaison between internal and external participants, visitors, and staff
- Train and conduct HIV testing and counseling
- Train and implement the CMS program with identified youth and make appropriate referrals as needed
- Fill out and be responsible for keeping logs and numbers of all participants in the education classes and outreach activities
- Engages in participant interactions via telephone and in-person to identify, advocate, and meet participant needs to ensure linkage and engagement to HIV care, substance misuse, and behavioral health services, including prevention services such as PrEP
- Attend PRCC and Public Health Initiatives (PHI) events as needed to provide support in administering screening questionnaires, intervention, providing referrals to PrEP, Behavioral or Substance Use Services
- Retrieves and enters information on assigned systems (EvalWeb, Excel, or other databases) and confirms and verifies participants’ current information
- Completes required documentation of services accurately, promptly, and thoroughly by the department standards to ensure documentation and reporting comply with all regulations
- Collaborates with a multidisciplinary team while also interacting with a diverse group of clients, visitors, and external and internal contacts
- Reviews all participant forms and ensures files are complete and in compliance with contract requirements
- Performs general clerical tasks as assigned, which may include but are not limited to filing, processing paperwork, data entry, scanning / photocopying, agenda creation, minutes, encounter reports, scheduling, and other similar tasks and activities
- Participate in ongoing training, staff development, and program evaluation activities
- Attends the PRCC and program-specific and organizational activities and meetings, including but not limited to the monthly organization-wide General Assembly
- Participates in organizational annual events including but not limited to the Puerto Rican People’s Parade, Fiesta Boricua, Three Kings Day, and Haunted Paseo Boricua
- Performs other functions assigned
This position requires compliance with all of The PRCC’s written standards, including its Standards of Conduct, all policies and procedures, and Regulatory Compliance requirements.
Compliance will be considered as part of the regular performance evaluation
Requirements : QUALIFICATIONS :
QUALIFICATIONS :
Familiarity with 13 18-year-old LGBTQ / MSM Latino and African-American population in the West Chicago Area and neighboring towns;
Experience providing outreach and working with underserved populations
- Knowledge of HIV (its physical and emotional effects) and community resources, preferred
- 1 year of experience working in HIV related field or social services, preferred
- HIV Test Counselor or any other HIV prevention training, preferred
SKILLS :
- Bilingual - English / Spanish preferred
- Must demonstrate excellent interpersonal, verbal, and written communications skills
- Demonstrated ability to build relationships, and work effectively with people of diverse backgrounds
- Exercise tact, objectivity, sensitivity, strategy, and judgment in dealing with a variety of people with a variety of co-occurring disorders
- Experience and knowledge of Microsoft Office software
LICENSURE :
IL Driver’s license and access to a privately owned insured vehicle
EDUCATION :
- High School Diploma or GED Equivalent
- Bachelor’s degree in social work, public health, science, business administration, or another related degree preferred
