The Registered Nurse is accountable for management and delivery of patient care to assigned patients in collaboration with the interdisciplinary team, and within the scope of practice delineated in the New York State Nurse Practice Act.
Provides direct patient care, delegates / assigns responsibility as appropriate to others, and serves as leader of the interdisciplinary team in collaboration with and under the direction of the attending physician.
Focuses on completion of the nursing process including patient / family education and discharge planning. Understands and acts appropriately to ensure optimum use of all resources.

- Assumes primary accountability for organizing and ensuring nursing care to a select group of assigned patients.
- Develops an overall plan of care throughout the episode with appropriate consideration of the continuum of care
- Supports and follows through consistently with plans of care established by other RNs with primary accountability.
- Collaborates with physicians and advanced practitioners for patient care needs to provide patient centric care in all settings using GFH Core Values as framework.

Responsibilities
- Establishes a therapeutic rapport with patients and caregivers and determines the patient’s perception of his or her immediate needs and concerns using GFH Core Values for framework.
- Identifies and documents the nursing diagnosis based on analysis of the health record and evidenced based practices utilizing clinical decision-support tools.
i.e., standing orders influenza and pneumococcal vaccine, telephone triage protocols, RN anticoagulation management, etc.)
- Defines expected outcomes from the diagnosis, involving the patient, family, provider, and healthcare team in formulating measurable expected outcomes within a defined timeframe
- Identifies and develops an ambulatory care plan for patients seeking care for health promotion, health maintenance, or health related situational problems (i.
e., conducts daily clinical huddle with care team to review gaps in care and plan for discussion and implementation with patient and family / caregiver).
- Facilitates care coordination across health care settings to maintain continuity of care to ensure patients and populations are directed to the appropriate level of care.
- Collaborates with physician and advanced practitioner providers to provide patient centric care using Glens Falls Hospital’s Core Values as a guide with all interactions.
