About The Position
ID : 51758629 Shift : Day 5x8-Hour (08 : 00 - 16 : 30) Description : In need of a LCSW with discharge experience required - would be willing to see experienced discharge Case Manager RN's.
MUST HAVE active Compact or WA RN in hand. This person will need to be independent problem solver, clinical social worker would be preferred, and must have complex discharge experience.
Must work well with the formal discharge planner on site and providing post care planning with the patient. Must work 2 of the 3 winter holidays. Unit notes : This is a new program in the state of WA. This position will be part of the complex discharge pilot / enhanced care management.
Preferred Experience in guardianship, working with DHS, brokering with outpatient, housing, homelessness.Special Requirements : local travel is required, may need to go between Colby Campus / Pacific Campus / other local shelters for patient care and follow up.

e. cell phone, pager, etc.) needed to perform the duties as assigned at no cost to Provider or Agency. If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of $50.
