About The Position
Job Title : Physical Therapist - Home Health Job Specialty : Home Health Therapy / Rehabilitation Job Duration : 13 weeks Shift : Day 5x8-Hour (08 : 00 - 17 : 00) Guaranteed Hours : Not guaranteed Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience License : Must have license in hand Certifications : CPR, BLS Must-Have : - Recent specialty experience - Proficiency with HomeCare HomeBase (HCHB) and Oasis within the last 6 months - Reliable transportation and a valid driver's license - Willingness to travel up to 75 miles from the assigned care center Job Description : - Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.- Conduct home visits and attend periodic office meetings.
- Manage an average of 5-8 patients per day, with productivity expectations of 30 points per week.- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Complete all documentation within 24 hours of patient appointments.- Travel time between patient homes is compensated, but the commute to / from the first / last patient is not billable.
- Adhere to a mileage reimbursement policy at the IRS standard rate.- Accept assignments in a range of territories, primarily in specific counties.
- Be open to floating to nearby clinics if the patient census is low.- Attend to territory needs created by workload and growth demands.
