About the Role : As an RN Care Delivery Field Supervisor, youll lead and oversee clinical services delivered directly to patients
homes, ensuring the highest standards of care. Youll work collaboratively as part of a dedicated management team, bringing guidance and support to field nurses and home health aides, and help deliver person-centered, high-quality care to participants.

- Supervise and support a team of Community Health Nurses (CHNs), Personal Care Aides, and Home Health Aides.
- Conduct joint visits and oversee care, ensuring compliance with individual care plans.
- Collaborate with the Interdisciplinary Team (IDT) to create and implement customized care plans for each participant.
- Provide hands-on education to participants and staff on best practices, safety, and healthcare topics.
- Take on a leadership role in new employee orientation and field visits, as needed. About You :
- Education : RN and BSN required (or a minimum of 5 years in a healthcare setting); Baccalaureate degree preferred.
- Experience : At least 1 year in home care or community health settings and experience with frail or elderly populations preferred.
- Licensure : Active, unrestricted RN license in New York State.
- Technical Skills : Proficiency in computer systems and managed long-term care insurance experience is a plus. Your Skills & Abilities :
