Description :

Ensure prescreen is filled out and attached upon submission.

Resume and license verification also need to be attached.

If these items are not completed and attached, the candidate will be declined.

Covid test to be performed 72 hours prior to the start date, with results issued 48 hours prior to the start date.

State license verification required.

Resume and three references are required.

County criminal background check covering the last seven years is required.

Federal criminal background check for seven years or more is required.

Multi-state criminal search covering seven years or more is required.

Nationwide sex offender search covering seven years or more is required.

HHS / OIG search is mandatory.

GSA search is required.

Social Security Number trace is mandatory.

One-step PPD or Quantiferon, or proof of positive PPD and Chest X-Ray is required.

SAM search is necessary.

Virginia State Police background check is mandatory.

Mandated reporter form is required.

Sworn statement submission is necessary.
