What You Will Do

Senior Helpers of Salt Lake has an opening for a compassionate, nursing professional to function as the Clinical Care Coordinator Nurse for our home care team.

This role manages our non-medical services, client care, supervises aides, conducts client nursing assessments, and develops, monitors, and revises client care plans.

Additionally, it assigns caregivers to specific clients based on recommendations. The position is responsible for conducting weekly orientation sessions for caregivers, assessing their skills, and offering clinical guidance and training.

This position operates on an as-needed basis with a competitive pay structure that varies depending on the setting office time or home visits.

Primary Responsibilities : (Duties subject to change, other responsibilities may be assigned)

  • Travel to clients' homes and complete all documentation for nursing assessments and observations.
  • Plan and complete regularly scheduled evaluations of clients and caregivers.
  • Evaluate clients regularly and / or as necessary to ensure client needs are consistent with the care plan.
  • Coordinate with management on implementing and revising operational procedures related to quality client care.
  • Conduct weekly orientation and training sessions for new caregivers and complete caregiver skills assessments to determine competency, and document.
  • Utilize professional assessment skills to identify the physical, mental, and psychosocial needs of clients and be able to develop a care plan to meet these needs in a multi-disciplinary approach.
  • Function as a resource and mentor to peers and subordinates to maintain professional nursing standards.
  • Act on clients’ behalf to ensure patient rights are observed.
  • Be aware of home safety issues, strive to accomplish all responsibilities safely, and consistently reinforce good safety attitudes and practices displayed by other staff members.
  • Document client complaints concerning caregivers and review complaints with the management team.
  • Coach all staff in proper safety procedures and consistently reinforce procedures.
  • Establish and maintain professional relationships with clients and their families; answer questions, respond promptly to any concerns, and promote services that will support individual client needs.

Qualifications We Require From You :

  • BSN, ADN degrees or Practical Nursing Certificate (BSN strongly preferred)
  • Current RN or LPN licensure ( RN licensure strongly preferred. LPNs only considered with 2+ prior years of home care experience and supervisory experience of CNAs)
  • BLS Certification
  • Ability to successfully pass a background check & pre-employment screening, including a current TB test.
  • Must maintain CEU requirements as required by the Board of Nursing
  • Valid Driver's License and reliable transportation
  • Able to comply with company policies, culture, and values and maintain confidentiality.
  • Able to communicate effectively with company staff, clients, and clients’ families.
  • Able to handle occasional physical and verbal mistreatment from clients professionally.
  • Able to supervise (instruct, assign work, etc.) caregivers while on duty.

Knowledge, Skills, and Attributes We Are Looking For :

  • A passion for senior care and service; finding ways to compassionately support clients and families.
  • Prior direct work experience of 2+ years in home health or home care (preferred for RN, required if LPN)
  • Prior experience supervising caregivers (home health aides and CNAs).
  • One year or greater experience working in geriatric care (preferred).
  • An independent and collaborative individual, driven and capable of self-direction.
  • Openness to adapt scheduling timing for nursing assessments, follow-up visits, and evaluations.
  • Proficiency in building rapport and fostering trust with both the internal Senior Helpers team and clients along with their family members.

Compensation and Benefits Available Depending on Hours Worked :
