Duties and Responsibilities :
- Completing accurate MDS assessments per RAI coding guidelines
- Initiating and maintaining care plans and supporting Interdisiplinary team as assigned
- Creating and distributing monthly care plan calendars in a timely fashion
- Monitoring & auditing clinical records, ensuring accuracy & timeliness
- Coordinating optimal quality patient care together with the IDT
- Protecting the confidentiality of Resident & Facility information at all times
- Tracking and optimizing CMS 5 star ratings and quality measures
- Closing out monthly billing timely and managing weekly utilization review meetings
- Other duties and essential functions of the MDS coordinator role as assigned
Requirements : Valid RN License
Valid RN License
MUST HAVE PRIOR MDS 3.0 EXPERIENCE
Long Term Care Experience Required!
Must be able to work 40 hours a week
Must be highly organized, professional & motivated
Should have solid computer skills
Excellent communication skills
Should be friendly and a team player
