In this role you will be working 4 nights aweek with a young adult client to provide specialized nursing carein their home and at their local summer camp.
Experience is notrequired; we will thoroughly train in this specialty and provideongoing support.
In this role youwill :
- Develop long termmeaningful relationships with the patients and theirfamilies
- Provide hightech nursing care totechnologydependent patients in theirhomes
- Collaborate with the healthcare team toensure comprehensive patient care
- Performassessments administer medications and educate patients andfamilies
- Utilize innovative technology tomonitor and manage complex medicalconditions
- Four 10hr shiftsnights
We are looking forsomeone who :
- Holds a currentLPN license in Vermont
- Two or more years ofdirectly related home / public health hospice and / or maternal / childhealth nursing experience
- Previous home careexperience a plus
- Possesses strong clinicalskills and a commitment to patientcenteredcare
- Demonstrates excellent communicationcollaboration and critical thinkingskills
