Job Summary :
The Hospital Case Management Implementation Coordinator (IC) is responsible for ensuring the successful integration of case management technology into hospital operations.
This role requires a deep understanding of both the software system and clinical workflows to optimize patient care, streamline processes, and enhance overall efficiency.
Key Responsibilities :
- Implementation Leadership :
- Drive the implementation of case management technology across multiple facilities.
- Coordinate pre-go-live activities, including training, testing, and data migration.
- Provide on-site support during go-live, troubleshooting issues and ensuring system stability.
- Assess implementation effectiveness and propose modifications as needed.
- Clinical Workflow Expertise :
- Collaborate with clinical staff to understand their needs and ensure the technology aligns with their workflows.
- Provide subject matter expertise on case management processes and software functionality.
- Support the development of training materials and deliver standardized education programs.
- Technical Proficiency :
- Demonstrate a deep understanding of the case management software system and its capabilities.
- Troubleshoot technical issues and escalate problems as necessary.
- Ensure consistent access and usage of the software among hospital staff.
- Team Coordination :
- Work closely with corporate, division, and facility leadership to align implementation efforts.
- Coordinate with the implementation team to provide necessary support to case management staff.
- Foster collaboration and communication among all stakeholders.
- Continuous Improvement :
- Analyze implementation outcomes and identify areas for improvement.
- Incorporate lessons learned into future implementations.
- Stay up-to-date on industry trends and best practices in case management technology.
Qualifications :
- Associate's degree preferred.
- Strong understanding of medical terminology and healthcare systems.
- Experience with case management software (e.g., Meditech, Cerner, Epic) is highly desirable.
- Excellent communication, organizational, and problem-solving skills.
- Ability to travel to multiple facilities.
