Dean’s Professional Services is now hiring a qualified Home Health Licensed Vocational Nurse to work in the Houston, TX area .

The Home Health Licensed Vocational Nurse is responsible for monitoring assigned cases to ensure compliance with the requirements of the third-party payor.
Listening and addressing patients’ concerns are an essential part of the job description for an LVN in-home health care.
Job Duties :
- Participate in planning and implementing care in conjunction with the RN, by the Plan of Care. Wound care, medications, and upcoming vaccinations could be included in the Plan of Care.
- Under the direction of the RN, assists in identifying the patient’s physical, psycho-social, and environmental needs as evidenced by documentation, clinical records, case conferences, team reports, and evaluations.
- Provide effective communication to patient / family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes
Job Requirements :
- High School Diploma Required
- Graduate of an accredited school of vocational nursing. - Required
- Must have Home Health Experience.
- Current Texas State license as a Licensed Vocational Nurse. Required
- 2+ years’ experience as a Licensed Vocational Nurse in a clinical care setting; home health Preferred
- Nursing skills as defined as generally accepted standards of practice. Must have good interpersonal skills.
- Reliable transportation. Valid and current Texas driver's license and valid and current auto insurance.
- Works in a patient’s home under various conditions, PRN emergency calls.
- Possible exposure to bodily fluids and infectious diseases.
- Must have the ability to work a flexible schedule and to travel the agency’s service area.
- There may be some exposure to inclement weather conditions.
Job Details :
- Schedule : Start Tomorrow
- Starting at $35 / hr. (based on experience) + benefits.
- Paid Mileage
