- LCSW preferred Must-Have : Experience in complex discharge planning Job Description : In need of an LCSW with discharge experience;
experienced discharge Case Manager RNs will also be considered.- The individual must be an independent problem solver and a clinical social worker with complex discharge experience.
- Must collaborate effectively with the formal discharge planner on-site and provide post-care planning with patients.- Willing to review first-time candidates who possess the right experience.
- Local travel required between various campus locations and possibly local shelters for patient care and follow-up.- Ideally have experience in guardianship, working with DHS, brokering with outpatient services, housing, and homelessness.
- Strong float capability between discharge locations is essential.- Commitment required to work 2 out of the 3 winter holidays.
- Expected to float between all facilities within the state; refusal may lead to termination of the assignment and / or forfeiture of guaranteed hours.
This is a new program focusing on complex discharge and enhanced care management, seeking a candidate who can contribute positively.
