PositionOverview : We are seeking a compassionate anddedicated Licensed Practical Nurse (LPN) to join our team inTrevose PA.
As an Aveanna Healthcare LPN you will have theopportunity to make a profound impact on the lives of our patientsand their families by providing highquality clinical care in a homehealthcare setting.

Responsibilities :
- Performpatient assessments and assist in developing individualized careplans under the supervision of a Registered Nurse(RN).
- Administer medications treatments andtherapies as prescribed by thephysician.
- Monitor patient progress and reportany changes in condition to the supervising RN orphysician.
- Provide patient and family educationon healthcare management medication administration and selfcaretechniques.
- Collaborate with theinterdisciplinary team to ensure coordinated and comprehensive caredelivery.
- Maintain accurate and uptodatepatient documentation in accordance with regulatory standards andorganizational policies.
- Provide emotionalsupport and advocacy for patients and their families throughout thecare continuum.
- Primarily hiring for 12 hourdays and 8 hourovernights.
Qualifications :
- Currentand unrestricted LPN license in the state ofPennsylvania.
- Minimum of six months of clinicalexperience preferably in a home healthcare or pediatricsetting.
- Strong clinical assessment andcritical thinking skills.
- Excellentcommunication and interpersonal skills.
- Abilityto work independently and collaboratively within amultidisciplinary team.
- Reliable transportationand willingness to travel to patient homes within the Trevosearea.
