Summary The VA Greater Los Angeles Healthcare (VAGLAHS) is seeking a highly skilled / experienced Registered Nurse to serve as Home Base Primary Care (HBPC) Program Director.

Demonstrates leadership, experience / ingenious approaches to management of complex patient care. Accountable for the management of patient care, human, fiscal / material resources, and the total environment of the unit.

Responsible to ensure that the highest possible quality of care is achieved and a safe therapeutic environment. Responsibilities THIS POSITION REPORTS DIRECTLY TO THE ACOS OF GEC Typical duties include but are not limited to : The Home-Based Primary Care (HBPC) Program Director is an RN and is accountable for the management, supervision, and evaluation of care for patients enrolled in the HBPC program.

The HBPC Program Director is responsible for directing the program's clinical and administrative services to ensure that the plan complies with local and VHA standards and policies as well as accreditation standards for home care organizations.

Responsible for the development and continued effective functioning of the interdisciplinary health care team. Collaborates with other HBPC Program Directors, Veteran Integrated Service Network (VISN) leadership, and VA Central Office staff as needed.

Responsible for interpreting and communicating VA policies to the HBPC team and the VHA facility. Responsible for the HBPC team development and implementation of local HBPC policies and procedures to include performance improvement, patient safety, utilization review, emergency preparedness, and staff safety.

Responsible for evaluating the program's effectiveness and ensuring effective / efficient resources are maintained. He / she assures the program can optimize cost-effectiveness (budget) and patient outcomes.

Participates in the formulation of overall service / program policies and procedures, goals, objectives, strategic planning, cost-effectiveness, and quality monitoring.

The Program Director for HBPC is responsible to aid, collaboration, and subject matter expertise for a complex group of services HBPC.

Responsible for evaluating program activities and providing follow-up actions required by internal and external review organizations and or accrediting bodies.

Responsible for communicating and interpreting non-VA national, state, and local home care policies and facilitates coordination and communication with non-VA care providers when necessary.

Facilitates an atmosphere of interactive management and the development of collegial relationships among nursing personnel and other team members.

Promotes a Just Culture environment in building a climate for effective performance while allocating available resources to promote safe, efficient, effective, and quality nursing / patient care.

Observes and evaluates the quality of nursing care provided including interdisciplinary patient rounds, reviewing nursing documentation in clinical records, and collecting, reviewing, and analyzing data related to the delivery of patient care.

Assure the compliance with all regulatory agencies and standards of practice ANCC and implementation of required actions to meet Performance Measures.

Provides leadership in the application of the nursing process to patient care, organizational processes, and systems, and improves outcomes at the program or service level.

Initiates an educational plan to meet changing program or service needs for self and staff. Provides leadership in identifying and addressing ethical issues with patients and staff, including initiating, and participating in ethical consultations.

Involved in functions such as recruitment; hiring; coaching; payroll processing and administering disciplinary policies.

Responsible for goalsetting, on-going measurement of goals, administering the recognition program, and all other human resources programs.

Contributes to the strategic planning process, day-to-day operations, Logistics Management, Budget, Staffing, Human Resource actions, Hiring and attainment of goals of the organization.

Qualifications Basic Requirements : English Language Proficiency. In accordance with 38 U. S. C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.

Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant : The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE);

OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program.
