Responsibilities of the Field Home Health Registered Nurse (RN) inJackson, MS :
- Document in real-timefor OASIS Visits : admissions, resumptions of cares, re-certifications, discharges, transfers, and routine visits
- Submit documentation within 24-48 hours
- Case manages 25-30 patients per week, 5-7 patients per day
- Provideeffective communication and serveas a liaison to patients / family, team members, administration, physicians and other health care professionals.
- Ability to work flexible schedule and travel locally in the Jackson, MS area
