Overview :
Responsible for providing assessment and referrals for individuals living with HIV to increase access to medical care and other needed resources.
Responsibilities :
- Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
- Performs assessment of individual needs and develops and documents appropriate plan of care
- Anticipates insurance lapses, assists with required insurance and medication coverage applications
- Assesses for related program eligibility, mental health, housing services, substance use services, and other entitlements
- Documents all required data into the reporting systems including CareWare. Provides reports as requested by Program Manager
- Provides ongoing follow up and evaluation of the effectiveness of services being provided
- Adheres to the procedures and documentation as required by Part B Standards of Care for Case Management
- Adheres to agency infection control and safety policies
- Adheres to the organization’s policy in regards to absenteeism and appearance
Qualifications :
- Bachelor’s degree in Social Sciences or related field
- 1+ years of HIV / AIDS work related experience
- Knowledge of cultural and lifestyle diversity issues
- Strong organizational and interpersonal skills
- Bi-Lingual Spanish preferred
