About The Position
Job Title : Pharmacist Job Specialty : Job Duration : 13 weeks Shift : 9 PM - 7 AM (7 on / 7 off rotation starting on Wednesday nights and ending the following Wednesday morning) Guaranteed Hours : 40.
00 hours per week Experience : - Acute Care / Hospital experience (must have 2 years hospital experience)- Cerner (EHR) - Pyxis (Automated Dispensing Machines)- Pharmacokinetic Dosing- Neonatal / Pediatric experience preferred License : TSBP Registration required Certifications : Pharm D required Must-Have : - English language proficiency : Speak, Read, Write Job Description : Review and process medication orders using the hospital information system and pharmacy computer system to ensure appropriateness in terms of dose, dosage form, route, frequency, duration, and patient's disease state.
- Supervise pharmacy technicians and interns to ensure pharmacy products are prepared and distributed accurately and timely.
- Prepare and distribute pharmacy products accurately when necessary.- Ensure medication orders are appropriate for age, weight, and disease state for 100% of orders.
- Review medication orders for potential allergic reactions, drug interactions, therapeutic duplication, and incompatibilities for 100% of orders.
- Ensure PRN medication orders contain indications and are documented 100% of the time.- Document reasons and rationale for orders outside the normal range in the pharmacy intervention record.
- Provide drug information and / or educational programs to healthcare professionals and patients.- Consult with healthcare professionals about the dose, dosage form, availability, administration, side effects, and incompatibilities of drugs.
- Advise patients and families on medication use including name, description, route, dosage, duration, directions, side effects, interactions, contraindications, self-monitoring, storage, refill information, and actions for missed doses as per OBRA.
- Provide training to new pharmacy staff in compliance with training goals / objectives.- Ensure compliance with pharmacy regulations and protocols.
