Primary Care Physician - Chicago, IL
Our client is a physician-led and mission-driven, primary care organization. They are currently one of the most successful full-risk Medicare Advantage providers in the nation and have a vision to be America’s leading primary care provider, transforming care of the neediest population.
Their mission is to honor seniors with affordable VIP care that delivers better health. In order to achieve their vision and deliver their mission, they need the best primary care providers and clinical leaders that are seeking to fulfill purpose and personal opportunity and join the team.
Our client offers the following :
- Compensation : Base $240k - $260k + bonus ($20K year 1 and goes up as a partner)
- Partnership (No Buy-In. Usually within 12-18 months. 65% have made partner after 1 year and partners will make over $300k)
- PTO : 33 days
- Health, Dental, Vision and supplemental benefits plans
- 401K - 5.5% match
- CME $3,500
Responsibilities :
- M-F 7 : 30- 5 : 00 PM (last patient scheduled a 4 : 15)
- Administrative time included in schedule to ensure work / life balance
- No nights or weekends
- 100% outpatient
- Small patient panel (450 max)

- Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
- Take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
- Leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.

- BC / BE in FM, IM or Geriatric Medicine
