Behavioral health facility seeking to fill a full-time case management position.
Requirements include :
- High school diploma or equivalent (Bachelor’s degree preferred)
- Valid Nevada driver’s license
- 1 year case management experience in a similar setting
- Must have excellent communication skills
- Must have experience with navigating electronic healthcare records platforms
- Must be proficient in computer skills, including Microsoft Office systems.
- Experience with Medicaid and local resources preferred
- Candidates must be available to work all shifts (days, nights, swing) and holidays.
- Must be able to pass preemployment drug screens and background checks, including a clean driving record
Responsibilities include :
- Assist clients in obtaining vital documents
- Setting appointments with various providers
- Assist clients with applying for NV Medicaid
- Work with community partners to collaborate care and services for clients
- Work in collaboration with treatment team and other departments to ensure client's needs are met
- Schedule necessary appointments for clients
- Assist in transporting clients to and from scheduled appointments
- Read and respond to a high volume of emails within an appropriate timeframe
- Adhere to Company policies and procedures
- Complete room checks and client UA's, when necessary
- Document client interactions thoroughly and timely
