Providence Medical Group (PMG) is seeking an experienced nurse practitioner or physician assistant to support our a team of internal medicine physicians at PMG Scholls Medical Plaza.
You'll be part of a collaborative, value-based, and patient-centered medical home made up of physicians, advanced practice providers, behavior health provider, clinical pharmacist, case management, and an experienced clinical support team.
A great opportunity for an experienced APP to see a broad range of complex and chronic medical conditions for adult and geriatric patients in a busy primary care clinic.
Just minutes from Washington Square, this community clinic in Tigard is in a fantastic location in the West suburbs of Portland, Oregon.
- Full-time, FTE
- Monday - Friday, daytime hours, 4-5 days / week schedule
- Non-paneled, team-based care with direct and indirect patient care (In Basket management)
- Ideal candidate will be comfortable seeing adult and geriatric patients and proficient with managing complex care coordination
- 2 years experience required
