Case Manager
The Case Manager plays a crucial role in supporting our participants on their journey to permanent supported housing. Our program is entirely participant-led, focusing on empowering individuals to self-identify their housing goals and determine the necessary steps to achieve them.
This role involves helping participants secure essential services, fostering an environment of mutual respect, empowerment, and opportunity to promote self-sufficiency within the community.
Key responsibilities include providing information on public services, educational opportunities, employment, and vocational skill supports.
The Case Manager assists individuals in recognizing their strengths and housing options. Additionally, duties include transporting participants to appointments, facilitating room inspections, retrieving birth certificates and social security cards, and accompanying participants on tours of prospective housing.

What you’ll get to do :
- Advocate : Speak up for people in the housing process.
- Mentor : Set goals and help people build confidence and relationships. Advise and inform individuals on community resources, including housing, employment, education, transportation, recreation, health, religious / spiritual, and other resources to meet their individual housing goals.
- Build Community : Ensure effective communication with people involved in the housing process, such as family members, case managers, and housing navigators.
Assist in developing formal and informal community support.
- Educate : Work alongside participants to teach skills such as housekeeping, bill paying, and self-advocacy with medical providers and service providers.
- Observe Confidentiality : Follow all rules of confidentiality relating to housing and medical information shared by the participant.
- Provide Referrals : Offer referrals for resources like housing, outpatient treatment, clothing, food, and overdose prevention training.
- Document : Record spending, client interactions, and goal setting.

Case Management and Navigating Wrap-around Services : You have experience with social service systems and providing comprehensive case management support to individuals facing complex challenges.
Demonstrated ability to connect clients with appropriate resources, advocate on their behalf, and facilitate access to vital services, ensuring their needs are met with compassion and efficiency.
About the Father’s Heart Street Ministry
The Father’s Heart Street Ministry is a Resource Center at 603 12th Street in Oregon City. Our goal is to help men and women find a safe refuge from homelessness, hunger, and addiction.
We began as a faith-based organization founded in 1999 that provides daily services and meals to all of our neighbors. While we are anchored in our faith, we are non-denominational, all are welcome to services regardless of religious conviction, if any.
Our program practices a low barrier housing first model, meaning that participants do not have to achieve sobriety upon or in the duration of their participation in this program.

Successfully complete a criminal background check.
Work Hours and Benefits :
- The weekly schedule is typically Monday-Friday from 9AM-5PM. Office hours are 11AM-4PM and staff are expected to be available on site to participants.
- Salary is competitive and includes benefits. Hours must be logged for verification purposes due to our non-profit status.
- Full-time employees are eligible for Medical, Dental, Vision and Life / AD&D insurance up to $25,000.
- The role is in person, requiring the employee to work on-site and to travel for meetings and task completion.
- Business-related mileage is a reimbursable expense covered by The Father’s Heart Street Ministry.
