Description

The Team Based Care (TBC) Manager RN independently uses the nursing process to care for a group of patients safely, therapeutically, and efficiently.

This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures / protocols and documented workflows.

The TBC RN collaborates with other team members to coordinate and execute the plan of care. Demonstrates initiative in the delivery of patient care in a Team Based Care Model.

Required qualifications :

  • Coursework / Training and Graduate from an accredited school of Nursing.
  • Upon hire : California Registered Nurse License
  • 2 years of Professional Nursing Experience

Preferred qualifications :

  • Bachelor's Degree in Nursing.
  • 1 year of experience in ambulatory care nursing

Providence Clinical Network (PCN) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care.
