Job Description :
- Provide professional nursing services in compliance with healthcare standards and government regulations
- Deliver skilled nursing interventions and rehabilitation services
- Work closely with RN Supervisor and physicians to update and implement care plans
- Maintain communication with patients, physicians, and healthcare team members
- Ensure accurate documentation of patient visits and shift notes
Qualifications :
- Graduate of an accredited LPN program
- Current nursing license and CPR certification
- At least 2 years of experience in a clinical setting, preferably in home health or hospice
- Valid driver's license and reliable transportation
Salary Range : $46,400 - $63,800 per year
